<p>FOR IMMEDIATE RELEASE &#8211; January 19, 2012 -Perfect Image Solutions, LLC is voluntarily recalling all lots of Men&#8217;s Minoxidil 15% Azelaic 5% Hair regrowth topical, 60mL; Men&#8217;s Minoxidil 10% Azelaic 5% Hair regrowth topical, 60mL; Men&#8217;s Minoxidil 5% Azelaic 5% Hair regrowth topical, 60mL; Women&#8217;s Minoxidil 3% Azelaic 5% Hair regrowth topical, 60mL; and Hair regrowth shampoo enhanced with Ketoconazole and salicylic acid, 180mL, to the consumer level. The products are deemed “unapproved new drugs” under the law and U.S. Food and Drug Administration regulations and may present potential health hazards.</p>
<p>Use of these topical products containing 15% and 10% Minoxidil have not been shown to be safe, and could pose a risk to the general public due to the possibility of systemic absorption. Skin abrasions or irritations, such as psoriasis or sunburn, could increase the systemic absorption of topically administered Minoxidil. Minoxidil 15% and 10% could cause low blood pressure, heart palpitations and associated cardiac symptoms. Azelaic acid contained in the topical products could make the skin where it is applied more sensitive to sunburn. Ketoconazole contained in the shampoo product could cause hair discoloration and abnormal hair texture, removal of the curl from permanently waved hair, itching, rash, skin irritation and dry skin. Salicylic acid contained in the shampoo could cause mild, temporary burning, itching, irritation, or stinging.</p>
<p>Perfect Image Solutions, LLC has not received reports of serious injuries associated with the products subject to this voluntary recall.</p>
<p>These products are used to slow or stop hair loss and promote hair regrowth and are packaged in glass-dropper containers or plastic shampoo bottles, and are sold in single units. They are used by men and women generally in middle to late adulthood. The affected product lots include the following lot numbers and expiration dates:</p>

<p>Saw palmetto with zinc and vitamin B-6 are taken mainly for two conditions: androgenic alopecia and benign prostatic hyperplasia, commonly referred to as BPH. Androgenic alopecia, also known as male pattern baldness, is the leading condition responsible for hair loss. The cause is believed to be a genetic one, and involves imbalances in testosterone levels. BPH refers to growth of the prostate in older men. When this gland enlarges, it can cause problems such as painful and frequent urination and an increased risk of urinary tract infections. Consult with your doctor before trying a new supplement.</p>
<p><strong>Androgenic Alopecia</strong></p>
<p>Saw palmetto, zinc and vitamin B-6 all appear to have a normalizing effect on androgenic hormone levels, and therefore have a potential for preventing or stopping androgenic alopecia. Although supplementing with zinc and B-6 can stop any hair loss that is caused by their deficiency, whether the use of either is effective in treating androgenic alopecia in otherwise healthy subjects is yet unclear. According to the January 2009 issue of the “Journal of Cutaneous and Aesthetic Surgery,” the efficacy of saw palmetto for this condition is also questionable.</p>

<p>There are a number of studies investigating the use of saw palmetto for BPH, and although there are still some conflicting findings, it appears that it is a promising therapeutic solution, as discussed in the 2004 issue of the &#8220;American Journal of Chinese Medicine.” There is less evidence regarding the efficacy of zinc and vitamin B-6 in the treatment of BPH, but most patients will take these together. The rationale is that zinc and B-6 are essential for normal hormone levels, and since according to the National Kidney and Urologic Diseases Information Clearinghouse, hormonal factors appear to be implicated in BPH, supplementing with these nutrients can be of help.</p>
<p>Other Uses</p>
<p>Preliminary evidence indicates that saw palmetto may be useful in the treatment for prostate cancer, as reported in the September 2007 issue of the “International Journal of Oncology.” However, further research is needed. Zinc is useful for persons suffering from zinc-deficiency symptoms, namely growth retardation, diarrhea and an increased susceptibility to infections. Similarly, vitamin B-6 is useful for persons deficient in it. Symptoms of vitamin B-6 deficiency include confusion and mouth sores.</p>
<p><strong>Safety</strong></p>
<p>Zinc and vitamin B-6 are safe when taken at recommended dosages &#8212; check the manufacturer’s label. This will be based on the recommended daily allowance. According to the June 2008 issue of “Complementary Therapies in Medicine,” saw palmetto was found to be safe at 160 mg twice daily.<br />

<p>CLINICAL PRESENTATION OF ANDROGENETIC ALOPECIA</p>
<p>Alopecia is a general term for hair loss and requires further description. Androgenetic alopecia (AGA) is the most common cause of hair loss, presenting as loss of hair over the top (vertex) and the anterior mid-scalp area (receding hairline) in affected men. The term androgenetic alopecia denotes that both a genetic predisposition and the presence of androgens are necessary to cause expression. AGA is also referred to as male pattern hair loss and typically begins gradually in men in there 20s with incidence increasing 10% per decade.</p>
<p>Presence of Androgens must be present for baldness to occur</p>
<p>For centuries, it has been observed that the presence of androgens was necessary for Androgenetic Alopecia to express itself. In 400 BC Hippocrates observed that eunuchs (castrated males) did not become bald. Aristotle noticed this also.(1) Through out history it was also observed that the Italian Castrati (boys that castrated in order to train them as adult soprano singers, a practice which was ended by Pope Leo XIII in 1878) never became bald. Researchers knew that AGA had to be associated with the male hormone testosterone. It comes as no surprise that current research shows that the balding scalp contains miniaturized hair follicles and increased amounts of DHT compared to a hairy scalp. This suggests that it is the excess presence of dihydrotestosterone in the scalp tissue that causes AGA in those patients genetically predisposed.</p>
<p>Pathophysiology of Androgenetic Alopecia</p>
<p>In the body testosterone is broken down by an enzyme call 5 alpha reductase to dihydrotestosterone. (DHT). DHT, a potent metabolite of testosterone causes a gradual, progressive shrinkage in the length and caliber of genetically programmed hair follicles. This process is called miniaturization. Miniaturization results from shortening of the anagen phase and a decrease in the sit of the dermal papilla and volume of matrix cells. Consequently, each succeeding hair cycle results in production of smaller, finer hairs which contribute less to the overall appearance and density of the hair. These biochemical events occur at the cellular level of the hair follicle. Because the dermal papilla is highly vascular, it is continuously bathed in circulating androgens. It has been demonstrated that the dermal papilla is rich in androgen receptors and is the primary target of androgen action. (Choudhry et al., 1996) Cells in genetically programmed hair follicles contain the enzyme 5 alpha reductase. 5 alpha reductase converts testosterone into the more potent DHT (Chen, Zouboulis &amp; Orfanos, 1996). 5 alpha reductase is found in higher quantities in the scalp follicles of affected men. (Sawaya &amp; Price 1997) Androgen receptors in the cells of the dermal papilla bind with circulating DHT, forming androgen receptor complexes. This results in the androgen effects of miniaturization on the hair follicle. (Randall et al., 1992)</p>
<p>In conclusion, by inhibiting the breakdown of testosterone to DHT, hair loss can be prevented or at least kept to a minimum.</p>
<p>Treatments for Androgenetic Alopecia</p>
<p>5 Alpha Reductase inhibitors</p>
<p>Drugs in this class work by inhibiting the enzyme 5 alpha reductase, which limits the conversion of testosterone to DHT (Chen et al., 1996) Finasteride (propecia) is the first drug in this class to undergo extensive clinical trials in men. Finasteride has selective activity against 5 alpha reductase. As a result, serum and follicular DHT levels are significantly reduced (Dallob et al., 1994).</p>
<p>Saw Palmetto extract</p>
<p>Studies have shown that saw palmetto is an effective anti-androgen. It acts in a similar way that propecia does. Firstly it lowers levels of DHT in the body by blocking 5 alpa-reductase. Secondly Saw Palmetto blocks receptors sites on cell membranes required for cells to absorb DHT. Although no studies have been carried out on saw palmetto and its relation to hair growth, studies have been performed on the use of Saw Palmetto in the treatment of benign prostatic disease, which is similar to androgenetic alopecia in that it also depends on the production of dihydrotestosterone. All of the studies that have been performed to date show that Saw Palmetto is an effective anti-androgen and has shown conclusively to be effective in the treatment of benign prostatic disease.</p>
<p>One may assume from this that since Saw Palmetto is an effective anti-androgen and is used in the treatment of prostatic disease then it may also be effective in the treatment of androgenetic alopecia.</p>
<p>Vitamins</p>
<p>Some vitamins have been show to inhibit the activity of 5 alpha reductase and subsequent production of DHT. These vitamins therefore may be of great benefit in the treatment of androgenetic alopecia or preventing baldness. There has been studies in which zinc is shown to inhibit 5 alpha reductase activity and it has therefore been concluded that zinc is beneficial in disease and disorders related to an excess in DHT. There has also been studies which have show that vitamin B6, Zinc and azelaic combined together even in low concentrations resulted in a 90% inhibition of 5 alpha reductase activity.</p>
<p>Recommendations</p>
<p>Propecia has been tested and found to be effective in the treatment of AGA. However, its list of possible side effects include sexual adverse experiences. It also affects PSA levels, which is the screening indicator for prostrate cancer.</p>
<p>Saw Palmetto has been proven safe to use. It has no known drug interactions and is well tolerated by most people. The only noted side effect in a very small percent of people is upset stomach. Saw Palmetto can be taken with zinc, vitamin b6, and azelaic acid for a synergistic effect.</p>
<p>Conclusion<br />
Normal healthy hair grows about ½ inch per month. It may take several months before any effects are noticed. Where the area is completely bald, hair may not grow, if the follicles are dead. It takes years for the hair to thin, so one must assume that it will take time to also reverse the process. Keeping the hair from further thinning is success in itself.</p>

				<content:encoded><![CDATA[<p><strong>Minoxidil and erectile dysfunction: Minoxidil in the treatment of male erectile dysfunction</strong><br />
Topically applied 2% minoxidil solution has been reported to increase diameter, rigidity and arterial flow to the penis. As a result it has been suggested as a possible treatment for erectile dysfunction. A total of 21 patients received 2% minoxidil for treatment of erectile dysfunction with instructions to apply 1 cc of the solution slowly over the glans penis 20 minutes before intercourse.</p>
<p>Average patient age was 52.5 years (range 29 to 65 years). The etiology of the impotence was neurogenic in 8 patients, vascular in 7, psychogenic in 4 and other causes in 2. Two patients also had clinical evidence of venous incompetence and 4 were diabetics.</p>
<p>One patient with psychogenic impotence noticed improvement in the duration of erection but no increase in rigidity or size after minoxidil application. One patient with impotence after excision of a Peyronie&#8217;s plaque reported a rigid erection adequate for intercourse after using minoxidil. This patient subsequently was able to achieve erections without using minoxidil. The remaining 19 patients had no improvement in erectile rigidity, or the ability to obtain or maintain an erection.</p>
<p>One patient did notice some mild burning on the glans penis after applying the minoxidil. No other side effects were noted in any patient. These results indicate that 2% topical minoxidil solution is not effective when applied to the penis in the treatment of erectile dysfunction. It is possible that a higher concentration, a different delivery medium or a different chemical composition may yield better results.</p>
